New Waiver process and fees for 2010-11:

Dear Parent/Guardian,

Next year the scholarship process for after care will be changing.  All current scholarships will end the last day of this school year, 2009-2010.  We will no longer be able to offer the carryover grace period for the 2010-2011 school year.  You will need to re-apply, and there is no guarantee you will be approved.  Scholarships will be limited and issued on a first come basis.  BASCC Registration for 2010-11 will begin August 2, 2010 in the school’s front office.

You will begin the school year paying the full amount, which is: $173.00 per child, and the registration fee of $25.00 per family.  We are letting you know now, so that you may plan for this additional expense.

To apply for the scholarship, you must first apply for free or reduced lunch as soon as possible.  The applications from Food and Nutrition Services will be available July 1, 2010.  You may obtain a copy at the school, print a color copy from the Internet at http://www.broward.k12.fl.us/foodservice, or pickup a copy at Food and Nutrition Services, located at 7720 West Oakland Park Boulevard, Sunrise, FL 33351 and complete the application in person.   You may call for an application to be mailed to you at 754-321-0250, this takes anywhere from 7-10 business days.

As soon as Food and Nutrition Services approves you for either “Free” or “Reduced” lunch, you will be issued a letter.  A copy of the letter must accompany your application for the scholarship.  In addition, you will need:

·       A signed copy of the student’s after school registration form.

·       A copy of most recent W-2 or Federal Income Tax (parent/guardian may cross out Social Security number when applying).

·       Proof of employment attached for all adults living in the household. 

·       Two most recent check stubs.

o   If applicant does not receive a check, a notarized letter from applicant’s employer stating days working and hours of employment by the company.  In addition, the letter must be on company business letterhead or a company business card must be attached.  The business information must have company name, address and telephone for verification purposes.

o   If applicant is self-employed, it should be noted on Income Tax submitted, and a notarized letter on company business letterhead or a business card must be attached. The business information must have company name, address and telephone for verification purposes.

o   Adults in school must attach their school schedule showing they are attending class during the hours after care is being provided. 

o   If there is an adult in the household who cannot care for child due to medical reasons, a note from the adult’s doctor must be submitted.
